It depends on why it was changed. Change it to make it more accurate - okay. Change it to hide malpractice - not okay, fraud.

Changing a medical record can be a very serious offense. As was said it is definitely fraud. If a doctor does it they can lose there license, and my not be able to practice medicine. If a non doctor does it im sure the authorities can file charges. Changing a medical record is not something to play with. Insurance companies can get involved as well as the law. Either way there is no positive outcome from it. Ethically, it is wrong, for obvious reasons. Honesty is always the best answer.
